At Fountain of Health Home Health Agency, we are dedicated to ensuring that our clients receive the highest quality of care. Services are provided in the comfort of our clients’ homes to individuals including adults and children who need personal care, to promote independent and healthy living in their homes.

Our home care providers have been trained specifically to provide compassionate assistance for your loved ones needs, using industry best practices, health, and safety standards. Before our caregivers are hired, they must each pass a detailed background check, and become licensed, bonded, and insured with our agency for your peace of mind. They must pass a training session and prove their capability to provide compassionate and professional care services.

Our agency provides and works with physicians, nurse practitioners, nurses, certified nurse assistance and home health aides to provide various healthcare services. Private duty nursing services are provided by licensed professionals and Certified Nursing Assistants under direct supervision of a Registered Nurse. Care is provided in accordance with a plan of care approved by a physician.

Team

registered nurses

Registered Nurses (RN):

Assess patients and provide care ordered by the Physician. Provide Case Management and Care Coordination including medical treatments, instruction on medications, disease processes, and wound care. Oversees and supervises client care plan and client care team.

Licensed Practical Nurses (LPN):

Under the supervision of the RN, provide client care including, but not limited to dressing changes, treatments, medication administration and management and observe client’s condition, as well as provide health training.

Certified Nursing Assistant (CNA):

Under the supervision of the RN or LPN, provides personal care to maintain hygiene and activities of daily living needs. CNA’s offer basic healthcare and help tend to the client’s health and hygiene in their living environment, including but not limited to, changing soiled sheets or bedpans, maintain the cleanliness and sanitation of the room or help feed a patient during meals.

Our CNA’s have limited medical training, but can monitor our client’s health, take vitals, track their symptoms, and can speak with clients about health concerns. Furthermore, they will report their findings to their supervising nurse.

Home Health Aides (HHA):

Under the supervision of the RN or LPN, provides personal care to maintain hygiene and activities of daily living needs. Offer basic healthcare and help tend to the patient’s living environment, including but not limited to, assisting with everyday tasks: bathing, grooming, restroom use, getting dressed, meal preparation, transportation, laundry, light housekeeping, running errands, and more

Our HHA’s are great helpers and companions to clients, but have limited medical training. so will rarely provide medical assistance. They cannot provide nursing care, and do not offer medical advice. However, HHA’s will monitor our clients’ condition and remind them to take their medication.

Medical Social Workers (MSW):

Provide counseling and assist with socio-economic difficulties that may develop around patient’s medical condition.

Physical Therapists (PT):

Evaluate strength, functional mobility, balance, and safety. Provide treatment including rehabilitative techniques for strengthening exercise, gait training, and muscle re-education to restore normal function.

Occupational Therapists (OT):

Evaluate strength, coordination and cognitive functional performance needed to carry out activities of daily living such as bathing, dressing, grooming, homemaking and leisure tasks. Provide treatment to improve functional performance, fine motor coordination, strengthening, home modification, and utilization of adaptive techniques or equipment.

Speech Therapists (ST):

Evaluate and treat speech and swallowing disorders, cognitive impairment and memory. Treatment provided consists of strategies for a safe swallow, communication including expression and comprehension, adaptation to tasks to accommodate cognitive abilities, and strategies for memory retention.
